Appointment Time Control
We respect your time and make every effort to
remain on schedule. Please try to keep your appointment. We have set aside
this time exclusively for you. If you're unable to keep an appointment, kindly
give us 24-hours notice. Otherwise, charges may apply. This courtesy on your
part makes it possible for us to give an appointment to another patient in
need of care.
Payment:
Payment is due when services
are rendered. We accept most insurance. You can call us and give your
insurance information. We will then call your insurance company personally to
verify that your insurance is accepted at our office. With most insurance
plans we only collect your estimated portion at the time of service.
We accept
cash, local checks, cashier's checks, money orders, Visa, MasterCard, American
Express and Discover cards as forms of payment.
Financing:
We provide financing through
Care Credit,
Healthcare Credit and
Capital One. Financing is subject to
approval and can usually be obtained within 30 minutes. If you would like to
use this form of financing on your first visit, you can stop by our office to
fill out the required forms before making an appointment.
